How to Enter: No purchase necessary. Entering or participating constitutes entrant's full and unconditional agreement to and acceptance of these Official Rules. The Dental Product Shopper Giveaway begins January 1, 2010 at 12:01AM Eastern Standard Time ("EST"), and ends December 31, 2010 at 11:59 PM EST (the "Giveaway"). You must be a citizen of the United States of America and at least the age of majority in your jurisdiction of residence at the time of entry to be eligible for this Giveaway. You may enter the Giveaway by visiting the Dental Product Shopper Remodel, Relearn, Relax webpage, located at http://www.dentalproductshopper.com/RRR, and providing the requested information. If you do not wish to visit the webpage, you may be entered by sending an e-mail during the entry period with your name, address, including zip or postal code, and telephone number, and the name and address of your dental office to: RRR@dentalproductshopper.com. Limit of one entry per person.

The Sponsor and its parents, affiliates, divisions, and agencies, and each of the foregoing entities' respective employees, officers, directors, shareholders and agents (the "Released Parties") are not responsible for incorrect or inaccurate entry information, human or electronic error, unauthorized human intervention, technical malfunction, lost/delayed data transmissions, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software or any combination thereof, inability to access web sites, personal injury, or property damage due to participation in this Giveaway, or for late, lost, incomplete, stolen, misdirected, mechanically reproduced, illegible entries or postage due entries and/or mail, or any other problem or error that may occur, including in connection with any prize.

Eligibility: To be eligible to win, you must be (i) a licensed dentist, (ii) a dental hygienist, (iii) a dental assistant, or (iv) an office manager that has been employed at a licensed dental office during the entire six (6) months immediately prior to entry. Employees, officers, and directors of Sponsor, its parent companies, affiliates, subsidiaries, third-party partners, advertising, promotion, and fulfillment agencies, their immediate family members and persons living in their same household, are not eligible to participate in the Giveaway. Void where prohibited. Federal, state, and local laws and regulations apply. Prizes will be awarded to the entrant's dental office, as identified by the entrant in his or her entry (the "Winner").

Random Drawings: Three Winners will be selected in a random drawing from all eligible entries received by Sponsor during the entry period. Sponsor's decisions on all matters pertaining to the Giveaway are final. Odds of winning depend on the total number of eligible entries received. The Winners will be notified by email or phone. Each prize Winner may be required to sign an affidavit of eligibility (which affirms that they have complied with these Official Rules) as well as a liability release, and where legal, a publicity release which if issued, must be completed, signed and returned within fourteen (14) days from date of issuance or the prize may be forfeited and awarded to an alternate Winner. Acceptance of a prize constitutes permission for the Sponsor to use Winner's name and likeness for advertising and promotional purposes without additional compensation unless prohibited by law. If, in the Sponsor's opinion (in its sole discretion), there is any suspicion or evidence of tampering or technological corruption of the Giveaway, or if the security, integrity or proper play of the Giveaway is compromised in any way, the Sponsor reserves the right to void any suspect entries and/or modify, suspend, and/or terminate the Giveaway and award the prize using all non-suspect eligible entries received as of such termination date. CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF SPONSOR IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW.

Relearn (If Chosen)
Winner will receive up to $10,000 for tuition reimbursement for continuing education at the institution of their choice within one year from the date of notification. Winner will also receive a $5,000 travel certificate paid for by Integrated Media Solutions, and issued by the Peak Travel Group; 1221 Lincoln Ave San Jose, Calif. 95125 / Toll-Free Phone: 800.737.9776 to be used for travel expenses related to courses approved by Sponsor. The travel certificate may be used only for the following: airfare and accommodations. Winner will submit invoice for approved educational program to Sponsor for payment. Sponsor will not pay for any late registration or other similar fees. All classes and travel must be complete within one year from the date of notification.

Relax (If Chosen)
Winner will receive a travel certificate paid for by Integrated Media Solutions, and issued by Peak Travel Group in the amount of $10,000 to be used exclusively for the following travel-related expenses: airfare, accommodations, select meals, and tour packages. The travel certificate is valid for travel anywhere in the world. All travel must be booked using the Peak Travel Group, 1221 Lincoln Ave San Jose, Calif. 95125 / Toll-Free Phone: 800.737.9776. All trips must be taken within one year from the date of notification. The winner will also receive a cash prize of $5,000 to use as spending money during the trip.

Second Place

One 2nd Place winner will receive a $2,000 cash prize.

Third Place

One 3rd Place winner will receive a $1,000 cash prize.

Any trip portion of prize must be taken on dates mutually agreed upon by Sponsor and Winner. If a Winner cannot travel on the dates specified, the prize will be forfeited. All travel arrangements must be made through Peak Travel Group. Airline ticket is not redeemable, has no cash equivalent, is not transferable or refundable and cannot be bartered or sold. If a winner lives within a 200-mile radius of the hotel or school designated by Sponsor, prize will not include the air-transportation portion of the prize. Except as otherwise provided in these Official Rules, Winner will be responsible for all gratuities, meals, beverages, ground transportation, taxes, and any other incidental costs or expenses not identified in these Official Rules. All tickets and accommodations are subject to the vagaries of schedule variation, work stoppages, and schedule changes.

WINNER PROCEEDS AT HIS/HER/ITS OWN RISK.

No prize substitution or transfer of prize permitted except by Sponsor due to prize unavailability, in which case prize substitution will be of equal or greater value. An unclaimed prize will not be awarded. After being contacted, via email or phone notification, a Winner will have fourteen (14) days to claim his or her prize. After the fourteen (14) days from time the email or phone notification is sent the Winner forfeits any right to claiming the prize. The prize is being offered are being provided "as is" and subject to the terms of the tickets. Sponsor will not replace any lost or stolen prize items. By accepting Grand Prize, Grand Prize Winner agrees not to resell or transfer the any potion of the Grand Prize to any third party for a period of 18 months from the date of award.

Disputes: Each entrant agrees that: (i) any and all disputes, claims and causes of action arising out of or connected with the Giveaway, or any prize awarded, other than the administration of the Giveaway or the determination of Winner shall be resolved individually, without resort to any form of class action, exclusively in the State or Federal Courts located in New Jersey; (ii) any and all claims, judgments and awards shall be limited to actual out-of-pocket expenses incurred, including costs associated with entering the Giveaway but in no event attorneys' fees; (iii) notwithstanding subsection (ii) above, it will release and hold harmless the Released Parties from and against any and all claims, actions and/or liability for any injuries, loss or damage of any kind arising from or in connection with participation in and/or entry into this Giveaway or use of any prize; and (iv) under no circumstances will entrant be permitted to obtain awards for and entrant hereby waives all rights to claim punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILTY FOR INCIDENTIAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules or the rights and obligations of the entrant and Sponsor in connection with the Giveaway, shall be governed by the laws of the State of New Jersey without regard to the conflicts of law provisions therein.

General Conditions: Giveaway entrants agree to abide by the terms of these Official Rules and by the decisions of the Sponsor, which are final on all matters pertaining to the Giveaway. Giveaway is void where prohibited or restricted by laws, including but not limited to jurisdictions with laws that would require registration, disclosure, translation, filing, trust account, or posting of a bond, or any other requirements that are not satisfied by these terms and conditions.
