How to Enter: How to Enter: No purchase necessary. Entering or participating constitutes entrant's full and unconditional agreement to and acceptance of these Official Rules. The Dental Product Shopper Giveaway begins January 1, 2009 at 12:01AM Eastern Standard Time (“EST”) and ends December 31, 2009 at 11:59PM EST (the “Giveaway”). You must be a citizen of the United States of America and at least the age of majority in your jurisdiction of residence at the time of entry to be eligible for this Giveaway. You may enter the Giveaway by visiting the Dental Product Shopper Ultimate Office webpage, located at www.dentalproductshopper.com/ultimateoffice and providing the requested information. If you do not wish to visit the webpage, you may be entered by sending an e-mail during the entry period with your name, address (including zip or postal code), telephone number, e-mail address (for prize notification) and the name and address of your dental office to: ultimateoffice@dentalproductshopper.com . Limit of one entry per person.

The Sponsor and its parents, affiliates, divisions, agencies, and each of the foregoing entities’ respective employees, officers, directors, shareholders and agents (the “Released Parties”) are not responsible for incorrect or inaccurate entry of entry information; human or electronic error; unauthorized human intervention; technical malfunction; lost/delayed data transmissions; omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software or any combination thereof; inability to access web sites; personal injury; or property damage due to participation in this Giveaway, or for late, lost, incomplete, stolen, misdirected, mechanically reproduced, illegible entries or postage due entries and/or mail, or any other problem or error that may occur, including in connection with any prize.

Eligibility: To be eligible to win, you must be a (i) licensed dentist, (ii) dental hygienist, (iii) dental assistant, or (iv) an office manager that has been employed at a licensed dental office during the entire six (6) months immediately prior to entry. Employees, officers, and directors of Sponsor, its parent companies, affiliates, subsidiaries, third-party partners, advertising, promotion, and fulfillment agencies, their immediate family members and persons living in their same household, are not eligible to participate in the Giveaway. Void where prohibited. Federal, state, and local laws and regulations apply. Prizes will be awarded to the entrant’s dental office, as identified by the entrant in his or her entry (the “Winner”).

Random Drawings: Three Winners will be selected in a random drawing from all eligible entries received by Sponsor during the entry period. Sponsor’s decisions on all matters pertaining to the Giveaway are final. Odds of winning depend on the total number of eligible entries received. The Winners will be notified by e-mail or phone. Each prize Winner may be required to sign an affidavit of eligibility (which affirms that they have complied with these rules) as well as a liability release, and where legal, a publicity release which if issued, must be completed, signed and returned within fourteen (14) days from date of issuance or the prize may be forfeited and awarded to an alternate Winner. Acceptance of a prize constitutes permission for the Sponsor to use the Entrant’s and the Winner’s name and likeness for advertising and promotional purposes without additional compensation unless prohibited by law. If, in the Sponsor’s opinion (in its sole discretion), there is any suspicion or evidence of tampering or technological corruption of the Giveaway, or if the security, integrity or proper play of the Giveaway is compromised in any way, the Sponsor reserves the right to void any suspect entries and/or modify, suspend, and/or terminate the Giveaway and award the prize using all non-suspect eligible entries received as of such termination date. CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF SPONSOR IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW.

All taxes and any other expenses are the sole responsibility of the Winner. No prize substitution or transfer of prize permitted except by Sponsor due to prize unavailability, in which case prize substitution will be of equal or greater value. An unclaimed prize will not be awarded. After being contacted, via e-mail or phone notification, a Winner will have fourteen (14) days to claim his or her prize. After the fourteen (14) days from the time the e-mail is sent or phone notification is made, the Winner forfeits any right to claiming the prize. The prize is being offered “as is” and is subject to the terms of the tickets. Sponsor will not replace any lost or stolen prize items.

Disputes: Each entrant agrees that: (i) any and all disputes, claims and causes of action arising out of or connected with the Giveaway, or any prize awarded, other than the administration of the Giveaway or the determination of Winner shall be resolved individually, without resort to any form of class action, exclusively in the State or Federal Courts located in New Jersey; (ii) any and all claims, judgments and awards shall be limited to actual out-of-pocket expenses incurred, including costs associated with entering the Giveaway but in no event attorneys’ fees; (iii) notwithstanding subsection (ii) above, it will release and hold harmless the Released Parties from and against any and all claims, actions and/or liability for any injuries, loss or damage of any kind arising from or in connection with participation in and/or entry into this Giveaway or use of any prize; and (iv) under no circumstances will entrant be permitted to obtain awards for and entrant hereby waives all rights to claim punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased.
