Case Study: Treatment of Malalignment with Clear Aligners
A 43-year-old female patient presented with a chief complaint of malalignment of her upper left central and upper left lateral. This patient had resisted seeking orthodontic treatment in the past due to financial concerns, but when MTM Clear Aligner (DENTSPLY Raintree Essix) was introduced, the price was right and very affordable for the patient.
Upper and lower polyvinyl siloxane impressions and bite registration were taken along with required pretreatment photographs and a panoramic x-ray to send to the MTM Clear Aligner Service Center (Figures 1 and 2). Upper and lower alginate impressions were also taken for in-office study models and orthodontic recordkeeping. The patient’s case was submit-ted with tooth No. 9 (UL1) and tooth No. 10 (UL2) as the target teeth, those needing alignment (Figures 3 and 4). If you are new to offering orthodontic treatment, the experts at the Service Center will assist you in identifying the target teeth and treatment planning your cases, including any necessary interproximal reduction (IPR). After approving the case online, the aligners were delivered in approximately 2 weeks.
Although there is no limit as to the number of aligners the Service Center will provide for completion of an accepted case, this patient’s case required only 3 aligners and 3 months of treatment time. The aligners came with a “clear aligner treatment plan summary” (Figure 3) The summary provided the areas where IPR needed to be done as well as how much tooth structure should be removed, and at what point in the treatment it should be done. Specifics as to the type of movement, which tooth, and what surface of the tooth each aligner was addressing were also provided in the summary.
Recommended wearing time of the aligners is 23 hours per day, except when eating or drinking anything other than water. Each aligner is worn for 3 to 4 weeks. Although a bonded lingual retainer was chosen for this patient, MTM Clear Aligner Service Center will provide a clear retainer at no additional cost. In 3 short months, this patient’s orthodontic treatment was completed using esthetic clear aligners (Figures 5 – 7).
