According to the American Academy of Dental Sleep Medicine, continuous positive airway pressure (CPAP) therapy is still the first line of treatment for sleep apnea, but many patients prefer an oral appliance to CPAP. Oral appliances help prevent the collapse of the tongue and soft tissues in the back of the throat, keeping the airway open during sleep and promoting adequate air intake. Oral appliances may be used alone or in combination with other treatments for sleep-related breathing disorders, such as weight management, surgery or CPAP.

According to Keller Lab's website, ClearDream is a dorsal appliance, as it uses ‘fins’ on the lower arch to advance the mandible. The ClearDream maintains an open airway using titratable posterior hardware in the upper arch to adjust the degree that the lower jaw is moved forward.
ClearDream is made using Keller’s proprietary, clinically unbreakable Crystal Clear 450 material. They also employ a wax/design/inject manufacturing process that allows the lower arch and fin to be fabricated in one piece, eliminating fin breakage.
ClearDream features an anterior opening to create additional tongue space, better oral airflow and increased patient comfort. The combination of the design and the Crystal Clear 450 material achieves sufficient retention, so no ball clasps are required for the ClearDream.
