Graduating from New York University College of Dentistry in 2011, she quickly established herself as a respected leader in the dental industry. As the owner, CEO, and operator of Secure Dental, a leading dental service provider with 10 offices across state lines, Dr. Jafri has over a decade of experience in successfully managing and growing businesses. For years, some of the patients who needed the most help were the ones Dr. Nazish Jafri simply could not treat. Today, the CEO of Secure Dental—an emerging DSO with offices in 4 states—cares for many of them in a single visit. Here, she shares how becoming certified in IV sedation lets her take on cases she would have missed, thanks to the training and clinical support she gained through DOCS Education.

There used to be a certain kind of patient I couldn't really help—patients with severe dental phobia, debilitating gag reflexes, or those who needed extensive, multi-quadrant surgical or restorative work who couldn't tolerate long appointments. I was routinely referring out 2 to 4 patients a month, some of whom never followed through with the referral and fell out of care entirely.

The Case That Stayed with Me

One patient made all this real for me. She was in her 50s and she hadn't seen a dentist in over 12 years due to severe past trauma. By the time she came to me, she had rampant decay, multiple broken teeth, and an intense sense of shame. We'd tried conventional, local-anesthesia-only treatment before, but it triggered panic attacks and made conventional care impossible.

She told me her granddaughter didn't want to hug her anymore. "Your teeth are scary, Grandma," the little girl said. That hit me hard. It took real courage for her to walk into my office, and I respected that. Without sedation, I know exactly how her story would have gone. She would have kept avoiding the dentist until an acute infection forced her into an emergency room for drainage.

Instead, we placed an IV line smoothly, titrated to a stable, moderate level of sedation, and completed extractions, placed implants, and delivered a full arch of temporary fixed teeth—all in the same day. Her vitals stayed rock-solid throughout. She had no memory of the procedure, and she woke up feeling relieved rather than traumatized.

Screening First

DOCS didn't just give me the ability to sedate—it gave me a new way of thinking about it. It shifted my mindset from "Can I sedate this person?" to an objective, safety-first screening framework. Airway and systemic health come first, with clear guardrails that give me absolute clarity on the ideal in-office candidate vs someone who belongs in an ambulatory surgical center or hospital setting.

What It's Meant for My Practice

Treatment plans that used to be stressful, chaotic, and spread across many visits now fit into predictable, high-value morning blocks. Comprehensive dentistry is completed in 1 or 2 appointments, and patients feel much calmer and more satisfied.

Sedation has also allowed me to treat people I never could before. We've unlocked a previously unreachable area of our local market: high-fear, high-need patients who now travel significant distances simply because IV sedation is offered in a comfortable general-practice setting. I'm grateful I get to be the one to help them.
