A standard prophy appointment requires an ultrasonic scaler with appropriate inserts for deposit removal, hand scalers and curettes for fine scaling, a prophy angle and paste for polishing, and fluoride varnish for remineralization. Desensitizers, biofilm debridement solutions, and between-teeth cleaning aids are added based on individual patient need.

Ultrasonic scalers use high-frequency vibration to break up calculus and irrigate the sulcus — faster for heavy deposits and large surface areas. Hand scalers and curettes provide superior tactile sensitivity for fine subgingival calculus removal and root planing. Most hygienists combine both for comprehensive debridement across a full appointment.

Interproximal alternatives including soft picks, proxy brushes, and water flossers have clinical evidence supporting their effectiveness for patients who resist traditional floss. GUM Soft-Picks and Proxabrush Go-Betweens evaluated by DPS hygienist panels showed plaque removal comparable to floss with significantly higher patient compliance rates reported at recall.

The standard in-office fluoride varnish is 5% sodium fluoride, which is significantly higher than over-the-counter fluoride products. It is applied directly to tooth surfaces, sets quickly on contact with saliva, and provides sustained fluoride release. Rosin-free formulations like Profisil are increasingly preferred for better patient tolerance and reduced allergy risk.

Independent panels of practicing hygienists and dentists use products in their own offices for approximately 4 weeks, rating on criteria including clinical performance, patient acceptance, ergonomics, and ease of recommendation. Products scoring 4.3 or above earn the DPS Best Product designation.
